DOT Exams: Review before calling

The process of getting a DOT exam can take several weeks in some circumstances, but usually it takes less than one hour.  

  • A full bladder for a urine sample
  • Glasses or contact lenses if you wear them
  • Your driver's license
  • List of medications
  • Any required special forms, completed by the managing provider beforehand

  • You take prescribed medication that is controlled, causes drowsiness, or relieves pain. The prescribing provider needs to complete this form.
  • your current vision with glasses is not 20/20
  • you have diabetes or a seizure disorder
  • you have sleep apnea. You need a letter from a sleep doctor stating you are compliant and therapeutic with your CPAP. Otherwise, you must be able to pull up CPAP software on your phone to show sleep/CPAP data during appointment.


  • You call and schedule the exam because you need a new DOT license or  it is close to 45 days before your current license expires. If you are uncertain of the status of your license, click here.
  • You tell us who is paying for the exam.
  • You are told to read this website and make sure you can be prepared for the exam by the scheduled date
  • You show up with any required forms from specialists, with a full bladder, glasses or contacts, driver's license, and a list of medications
  • You check in on the first floor because that is where you will give a urine sample. You give a urine sample.
  • You check in on the first floor and pay for the exam unless your employer or school has made other arrangements to pay for it.
  • You will be given forms to complete in the reception area.
  • The nurse will call you back, take your vitals, test your peripheral vision, visual acuity, monocular acuity, and color perception, and collect your completed forms.
  • Dr. Gallus will review your forms and examine you, including a hearing test (usually).
  • Dr. Gallus will make a determination.
  • If anything is required of the patient, that will be clearly communicated.
  • The forms will be completed and signed if the certificate is granted.
  • The forms are given to the patient.
  • The front desk makes copies of the forms.
  • The forms will sent to the  DOT registry and scanned into the chart. 

PLEASE NOTE: Ultimately it is your responsibility to  make sure the that the Montana Motor Vehicle Division received your renewal paperwork.
