Iris through Aledade for certain patients with Medicare: Iris is a company that helps people complete Advanced Care Planning. A very small subset of patients with Medicare are eligible, and we encourage those patients to work with Iris.

When the Advance Directives paperwork is complete:

 Bring your completed living will packet to your doctor to talk through your choices and have it added to your chart. Ask whether a POLST or MOELI is appropriate for your health needs, or let your doctor know if you want one in place. The Montana End of Life Registry  provides simple steps for uploading your documents. Make sure you share your Advance Directives with your family and with anyone you’ve chosen to act as your medical power of attorney.
