Living Will Programs

Five Wishes: this is the source of the booklets we used in class. 

Five Wishes Booklet for Catholics  in the Five Wishes store.

Six Steps Living Will: this is a more involved, detailed living will program. If you enjoy writing, this one might suit you.

The Conversation Project: many resources for individuals & families regarding ACP.

Iris through Aledade for certain patients with Medicare: Iris is a compay that helps people complete Advanced Care Planning. A very small subset of patients with Medicare are eligible, and we encourage those patients to work with Iris.

Health Care Power of Attorney

Montana State University provides information about Health Care POAs.

The American  Bar Association provides legal advice regarding all aspects of advanced care planning.

Healthcare Provider Orders for Life-Sustaining Treatment

These orders are created by you and your healthcare provider and must be signed by your healthcare provider. 

Montana POLST

Article about the MOELI form and an embedded copy of the printable MOELI

When the Advance Directives paperwork is complete:

Take your completed living will packet to your doctor to discuss and scan into your chart. 

Ask your doctor if your health situation warrants a POLST or MOELI, or tell them that you want one in place. The discussion will be worthwhile, even if the answer is no.

The Montana End of Life Registry provides an overview and easy steps to upload your documents. 

Be sure you discuss your Advance Directives with your close family and anyone you have chosen as a medical power of attorney.

This is the free will creation site that is recommended by PBS. It is user friendly and easy to complete.

Tying up loose ends:

Getting organized for death is big business and many agencies are available for hire to help you.

Look through the linked National Institute of Health webpage to help you decide if you can do it yourself for free.
